For Potential New Patients
If you are NOT a current patient of Transition Physician, and currently have a Home Health Agency coming to your home, you may be a candidate for Transition Physician care.
If you are interested in receiving physician services from us, please contact your Home Health Agency and ask them to contact us directly. If they are not familiar with Transition Physician, tell them to go to www.transitionphysician.com and follow the instructions below.
If you are NOT a current patient of Transition
Physician, and have significant difficulty traveling to your Primary
Care Physician's office, you may be a candidate for Transition Physician care.
If you are interested in receiving physician services from us, please send your request and a brief description of your situation to the email address below - or - go to the Contact Form Page and fill in the contact form.
If you are a current patient of Transition Physician, it is important that you first call your Home Health Agency Nurse and inform her/him of your concern. He or she will determine your initial needs, then, if necessary, contact Transition Physician for additional advice or to schedule a home visit for you.
After talking with your Home Health Agency Nurse, if there is any question as to whether you need to be seen by Transition Physician, we will call you personally and discuss your needs to make that determination.

Transition Physician sees your loved one in his/her home
within 24 to 72 hours following discharge from a hospital or rehab
facility to establish care and determine a Plan of Care for their
continued healing process. Following their initial visit, we will make scheduled visits to assure they continue to progress throughout
their transition to independence. Visits will be scheduled
based on their needs and are not limited. The particular needs
of your loved one will determine how frequently they are seen by the

will be called by the Home Health Agency nurse and a visit will be
scheduled as early as the same day depending on the situation (see Current Patients section above).
