For your convenience, we will bill your insurance company for you. If you have a secondary insurance or Medicare supplement plan (MediGap), we will bill them for you as well.
Please bear in mind that insurance plans are constantly changing with new policies, provisions, rules, restrictions, etc. Also, insurance companies offer many different plans with different names that change frequently. Therefore, we recommend you contact your insurance company for up-to-date plan coverage.
We will do our best to comply with the requirements of your insurance plan. In order to help you maximize your benefits, please bring your current insurance card to every visit. If you have any questions, please feel free to call our office.
We currently accept the insurance plans listed below. If your insurance company is not listed below, that does not necessarily mean we do not participate in your plan. ("Participation" means that we are an "in-network" provider for your plan.)

*Notice: Many Medicare Advantage plans are regional, which means that we may or may not be in-network for your plan, depending on where you live. Always, always check with your plan to be sure.

PLEASE READ THIS: If your insurance provides a no-cost annual wellness/preventive visit and you want to use that benefit as your first visit, please make sure our office knows that BEFORE you come in. If you have already scheduled your first visit, please call our office as soon as possible and tell us that you wish to change your first visit to a "new patient well visit." We are not able to verify who is or isn't eligible for a well visit from their insurance company, so please check with your insurance company before scheduling a well visit. Thank you.
